So, I know how to get the equation from the zeros, but I am confused with what I am supposed to do with " f ( 5 ) = 15".

And since f ( 5 ) = 15 , then 25 a + 5 b = 15 which means (after simplifying) 5 a + b = 3. Solve this for b to obtain b = 3 5 a. Substitute this into 36 a + 6 b = 0, to obtain

36 a + 6 ( 3 5 a ) = 0 36 a 18 30 a = 0 6 a = 18 a = 3
